Terms of Service

1. What VACstatus SmartForm is

VACstatus SmartForm ("SmartForm", "the Service") is a free tool for building medical-device product forms. It auto-fills device fields from public FDA databases (openFDA / AccessGUDID), lets you complete the rest, and exports a spreadsheet. By using the Service you agree to these Terms.

2. Product data only — no patient information

The Service is built for medical-device product data — catalog numbers, descriptions, pricing, units of measure, and similar commercial fields. It is not designed, intended, or permitted to store Protected Health Information (PHI), patient records, or any information about identifiable individuals' health or care. You agree not to enter patient information into any field, form name, or file you upload. VACstatus is not a HIPAA business associate, does not sign BAAs, and processes no patient data by design.

3. FDA data — no warranty of accuracy

Auto-filled fields come from public FDA device databases. These sources can be incomplete, out of date, or wrong, and our matching can make mistakes (ambiguous catalog numbers are flagged for your review). FDA-sourced values are provided as-is, for convenience — you are responsible for verifying any data before relying on it for procurement, regulatory, clinical, or any other purpose.

4. Form links and sharing

Every form has a unique link, and anyone with the link can view and edit the form — that is how sharing works here, like a shared spreadsheet. Treat form links as you would the spreadsheet itself: only share them with people who should see the contents. Forms created while signed in belong to your account; only the owner can delete an account-owned form.

5. Acceptable use

• No patient information or other unlawful, infringing, or harmful content.
• No attempts to probe, overload, or disrupt the Service or access data you were not given a link to.
• No automated scraping or bulk extraction of the Service beyond ordinary use.
• No reselling or misrepresenting the Service as your own.

We may suspend or remove content or accounts that violate these Terms.

6. Your content

You own the data you enter or upload. You grant us the limited rights needed to operate the Service — storing your forms, running FDA lookups on your catalog numbers, and generating your exports. We may use aggregated, de-identified usage information to improve the Service.

7. Accounts

You can use SmartForm without an account; anonymous forms live in your browser and at their links. If you create an account, you are responsible for safeguarding your credentials and for activity under your account.

8. The Service is provided "as is"

SmartForm is free, beta software. It is provided "as is" and "as available," without warranties of any kind, express or implied — including fitness for a particular purpose and non-infringement. We do not guarantee uptime, data retention, or error-free operation. Keep copies of anything important (the Excel export is yours).

9. Limitation of liability

To the maximum extent permitted by law, VACstatus and its operators will not be liable for any indirect, incidental, special, consequential, or exemplary damages, or any loss of data, profits, or business, arising from your use of the Service. Our total liability for any claim is limited to one hundred US dollars ($100) or the amount you paid us in the past twelve months, whichever is greater.

10. Changes and termination

We may modify or discontinue the Service, and may update these Terms — material changes will be reflected by the date above. Continued use after a change means you accept the updated Terms. You can stop using the Service at any time; deleting your forms removes them from the Service.
